What to bring
- Dental and medical insurance cards.
- Parent or guardian photo ID.
- Medication list and allergies.
- Any referral notes, dental records, or recent X-rays.
- Custody or guardianship documents when applicable.
We file claims for every insurance, including out-of-network plans. If your child has Renaissance or TennCare, have the child's Social Security number available when scheduling so benefits can be checked correctly.
No dental insurance? Ask about Smile Club, our in-office membership plan.
Keep the explanation short and positive. Tell them the team will count teeth, take pictures if needed, and help keep their smile healthy.
Bring a full medical history, medications, allergies, and any specialist information. Sedation instructions are case-specific and will be reviewed directly with you.
We still need Magnify/current form ownership confirmed before launch. Until then, call the office for the safest current paperwork path.
If something feels off after a visit, or if you are not sure whether your child needs to be seen, call the office. For severe swelling, trouble breathing, uncontrolled bleeding, or a major injury, seek emergency care first.
